Bring the actual plant or insect with
the form to the Purdue Extension - Grant County office at
401 S. Adams St.,
Marion, IN, 4th floor.
The office is open from 8:00 a.m. - 4:00 p.m., Monday
through Friday.
(Completing the information form is optional, but the information will be
helpful if the Educator is not in the office to talk with you at the time of your
visit.)
Important note: Some samples may require submission to Purdue Plant and Pest Diagnostic Laboratory. In these cases, the physical sample should be brought into to the Extension office (fresh or "actively dying" material with several good portions, or insect in identifiable condition either dead or alive). An $11 fee ($22 for out-of-state residents) will be required for each sample submitted. Additional fees may be required if specialized testing is needed (see website for details).

Note on mushroom identification: Some mushrooms are poisonous! Don't eat unless you know! If in doubt, identification by a professional mycologist is recommended. Sample submission to Purdue Plant and Pest Diagnostic Laboratory is highly advised.
