Silver & Golden Clover Application - due September 30th (disregard September 20th date previously published) - download and complete the fillable form below, or complete the web-based online form here.
4-H Club Information Form - due October 1st - download and complete the fillable form below, or complete the web-based online form here.
For 4-H Project Superintendents:
4-H Project Committee Form - due by February 21st - download and complete the form below and return to our office via mail or email (hartm115@purdue.edu), or complete the web-based online form here.
Workshop Sign-In Form - Please use this form to record workshop attendance. Only 4-H members need to sign in. The information gathered is used in our annual federal reporting to maintain the support of the 4-H Program. Please return these forms to our office via mail or email following each workshop.
Superintendent Commitment Form - Please read the document below entitled "2019-20 Superintendent Re-Commitment Info" before completing the form at http://bit.ly/2020SuptCommit. If we do not receive your response by February 21st, we will assume you wish to resign your position and we will proceed to appoint a new volunteer to serve as superintendent.
