4-H Club Financial Reporting Documents

• 4-H Club Annual Reporting Checklist - A helpful resource to use when meeting with Club Leaders annually.
• 4-H Club Confirmation- Target submission by October 1st to activate club in 4-H Online.
• Annual Club Plan - A tool to help Club Leaders plan out their meetings for the year.  Due no later than after the first club meeting, date to be set by county 4-H Professional.
• Annual 4-H Club/Unit Financial Report - Form to complete and return to the 4-H Educator summarizing the 4-H Club’s financial activity for the year.  Due by February 1st.
• Unit Activity Report Form - Report the planned activities of the 4-H Club on this form and submit the form to the County 4-H Extension Educator, prior to the time the activities are held.  Due by February 1st.
• 4-H Club/Unit Financial Review Form - Form to complete and submit during the year that the 4-H Club/Unit has been selected to submit to a more extensive financial review.  Financial reviews to be completed once every three years, annually if a club account is over $50,000, during a change in club leaders/primary volunteer working with the club finances or for special circumstances as designated by 4-H Professional.
